Tell Me a Tale Registration Form

Enrollment is Quick & Easy! (If you prefer to use a printable, mailable form, please use one of these links: Waunakee or Fitchburg.) Class availability is limited. Filled on a first-come, first-served basis. Minimum of 4 children needed to form a class. Please indicate your 1st, 2nd, and 3rd choices if possible. Summer class dates: 5-week session meets 2 hours once per week. 1-week sessions meet 2 hours daily, Monday through Friday. Parent/caregiver attends last 20-30 minutes of class. Waunakee: June 15 - July 24 (5-week session) July 27-31 (1-week session) Registration Fee: $59 reserves your place and purchases your home materials. Home Materials include: Home CD; hardbound literature book; parent's poster with listing of daily activities; and pair of lummi stick instruments. Includes snack and daily craft. NOTE: Non-refundable as your materials are purchased upon enrollment. Camp Tuition: $105 for 5 weeks. Payment plans available. Tuition not due until classes begin unless on payment plan. Total cost = Tuition + Registration
